Job Summary

Baptist Health is seeking a dedicated and compassionate Advanced Practice Provider (APP) to join our team in a remote capacity supporting patients across Jacksonville, Florida. In this role, you will provide high-quality, patient-centered care by evaluating, diagnosing, and managing a variety of health conditions.

The APP will collaborate with a multidisciplinary team to deliver evidence-based care, improve patient outcomes, and support Baptist Health's mission of providing exceptional healthcare to the communities we serve.

What You'll Do

• Conduct comprehensive patient assessments, including medical history and physical examinations

• Order and interpret diagnostic tests and lab results

• Develop and implement treatment plans, including prescribing medications

• Provide patient education on disease prevention, wellness, and treatment options

• Collaborate with physicians and care teams to ensure coordinated care

• Document patient encounters accurately in the electronic medical record (Epic)

• Participate in quality improvement initiatives and clinical best practices

Qualifications

• Master's degree in Nursing (MSN) or related advanced practice degree

• Certified Nurse Practitioner (AANP or ANCC)

• Active Florida APRN license (or eligibility)

• DEA Controlled Substances Registration

• Current, valid driver's license

• Strong clinical judgment and communication skills

• Experience in telehealth or remote care preferred
