The Community Living Center (CLC) Medical Director provides patient care directly or through the supervision of medical trainees in either inpatient or outpatient settings. In addition to their role as clinician, the CLC Medical Director will have administrative responsibilities for leadership and direction of CLC development and programs, as well as effective interface with the GREC administration. The CLC Medical Director will have clinical oversight during the CLC screening process. The incumbent will provide direct patient care in the CLC or outpatient setting to adults with acute and chronic medical conditions along a continuum of CLC care including skilled nursing, long term care, hospice/palliative care, and rehabilitation. The incumbent evaluates and manages adults who have single or multiple disorders in all subspecialties in the broad field of medicine and specifically geriatric medicine. The incumbent must be capable of working in interdisciplinary teams to provide comprehensive patient care. The CLC Medical Director will be expected to provide both administrative and clinical leadership, as well as direct patient care, in the CLC. The CLC includes skilled nursing, long term care, hospice/palliative care, and rehabilitation. Responsibilities include but are not limited to the following: Participates in the creation of policies, procedures, and guidelines to assure provision of quality comprehensive services. Collaborates with the GREC service regarding adequacy and appropriateness of scope of service. Mann-Grandstaff VA Medical Center Provides direct patient care including admission H&P, all day-to-day aspects of patient care, discharge summary, participation in discharge planning and any family conferences. Occasionally may provide patient care outside of the CLC, depending on Division needs: community nursing homes, Veterans' homes, and outpatient clinics. Assures that all Do Not Attempt Resuscitation orders in the CLC are countersigned by a CLC attending provider. Arranges for routine and unscheduled medical coverage in all CLC units. Develops rules and regulations for all the physicians who attend residents of the CLC. Collaborates with the CLC Screening Committee to ensure safe and appropriate CLC admissions, maximize the utilization of CLC beds, and maintain transparency in the admission process. Supports the CLC medical teaching mission in coordination with the Medical Facility and any outside teaching organizations. Supports research projects that may be instituted at the CLC. Collaborates and participate in the development of policies and procedures to assure compliance with the Long-Term Care Institute standards and the Commission for the Accreditation of Rehabilitation Facilities. Participates actively in the monitoring of quality and performance measures within the CLC units for compliance with standards, performance measures, patient safety, and other VA regulations. Provides guidance and leadership for various clinical-administrative committees, chairing committees, task groups or special project groups, and quality management/quality of care as appropriate. Provides input and recommendation to ECMS for credentialing and privileging of CLC providers. O. Works to assure a safe and sanitary environment for residents and personnel. Identify hazards for health and safety by reviewing and evaluating any incident reports. Complies with EEO/Affirmative Action and Prevention of Sexual Harassment policies for the station. Participate in opportunities for public speaking, leading site visits, and advocating for CLC at the facility, VISN, and VACO levels.
