The VISN 10 Clinical Resource Hub is recruiting for six Physicians (Primary Care). The Physicians (Primary Care) will function virtually within the VISN 10 Clinical Resource Hub, a repository of clinical & administrative staff that serve VA facilities within VISN 10 that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Provides direct patient care full time and works as a member of an expanded healthcare team (social worker, dietician, clinical pharmacists, behavioral health clinician, and a care manager) to oversee and coordinate medical care for a population of Veterans in a group practice setting utilizing the PACT model. Provides comprehensive care for Veterans for a panel size of approximately 900 patients or serves walk-ins. Performs initial histories and physical examination on outpatients using CVT technology and/or face-to-face visits. Provides ongoing outpatient care for assigned patients. Screens outpatients to determine the need for further care. Prescribes medications within defined scope of practice. Orders diagnostic studies such as laboratory tests, x-rays, electrocardiogram. Reviews need for and orders health promotion and disease prevention measures. Initiates, responds to and expediates requests for consultations, special tests and studies Provides education and counseling of patients and families in preventive care, medical conditions, and use of prescribed treatments and drugs. Serves as an attending/teaching physician for internal medicine residents/students as assigned. Attends staff meetings, team meetings, some committee meetings, and completes some peer review evaluations. Work Schedule: Full time, M-F, 8am - 4:30pm, or 7:30am - 4pm depending on spoke site needs; Evenings and weekends may be required as necessary. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
